If you are moving out of the area, please stop by our office to request a copy of your child’s shot record.
Once you move out of the area, our providers will be unable to refill your child’s prescriptions. Please contact a new provider as soon as possible to maintain your child’s medication.

New patients are required to come into our office and fill out a “New Patient Packet” prior to scheduling an appointment. Parent’s must bring the following…

the child’s insurance card
the child’s shot record
